Clinical Nurse Coordinator – Brain Health

About the role

The Brain Health Clinical Nurse Coordinator (CNC) leads patient‑centered care for individuals with cognitive disorders such as Alzheimer’s disease, vascular dementia and mild cognitive impairment. Reporting to the Cognitive Neurology Service Lead, the CNC acts as the primary liaison for patients, families and multidisciplinary teams within a specialised Neurological Institute.

Key responsibilities

• Perform comprehensive nursing assessments and develop care plans for patients with neurocognitive disorders.
• Coordinate diagnostic pathways, including laboratory work‑up, neuroimaging, neuropsychological testing and specialist referrals.
• Administer and document cognitive screening tools (e.g., MoCA, MMSE) according to service protocols.
• Schedule and manage outpatient Brain Health clinics, ensuring continuity of care and timely follow‑up.
• Provide education to patients and caregivers about diagnosis, disease progression, treatment options and community resources.
• Facilitate multidisciplinary collaboration among neurology, psychiatry, geriatrics, neuropsychology, social work and allied health professionals.
• Maintain accurate clinical documentation in EPIC and monitor patient pathways to remove barriers to care.
• Support advance care planning, quality improvement initiatives, research participation and compliance with regulatory standards.

Required profile

• Minimum 5 years of clinical experience in Neurology, Geriatrics or Critical Care.
• Strong knowledge of dementia and other neurocognitive disorders.
• Excellent communication skills for interacting with patients, families and multidisciplinary teams.
• Ability to manage complex care pathways and coordinate multiple services.

Required skills

• Proficiency with the EPIC electronic health record system.
