Clinical Social Worker – Patient Support

About the role

The Clinical Social Worker provides comprehensive psychosocial support to patients, families and caregivers within a medical setting. The role involves assessing needs, planning individualized care and advocating for the patient’s wishes throughout their treatment journey.

Key responsibilities

• Conduct thorough person‑centered psychosocial assessments and develop tailored care plans in collaboration with multidisciplinary teams.
• Supervise and mentor junior staff, ensuring adherence to departmental policies and standards.
• Manage a dynamic caseload, prioritising competing demands and allocating workload efficiently.
• Maintain accurate clinical records, including electronic data entry and report writing, in line with JCI requirements.
• Advocate for patients and families, ensuring their expressed wishes guide long‑term care decisions.
• Liaise with external agencies, government bodies and charities to coordinate comprehensive support services.
• Promote anti‑discriminatory practice and raise awareness of social work perspectives among colleagues.
• Uphold confidentiality and professional codes of conduct in all interactions.

Required profile

• Bachelor’s degree in Social Work (or equivalent) plus a postgraduate qualification (Master’s or higher) in Social Work.
• Previous experience delivering direct patient care in a medical or healthcare environment.
• Ability to work under high emotional stress while maintaining concentration and attention to detail.
• Strong understanding of patient rights, cultural values and ethical advocacy.
