Physiotherapist Post-Acute Rehabilitation

About the role

The Post‑Acute Rehabilitation Physiotherapist will deliver specialised physiotherapy services to patients admitted to the hospital’s post‑acute rehabilitation unit. This role focuses on restoring mobility, functional independence and quality of life after acute illness, injury, surgery or neurological events.

Key responsibilities

• Conduct comprehensive assessments for patients referred to post‑acute rehabilitation and develop individualized, evidence‑based treatment plans.
• Provide physiotherapy interventions to improve strength, balance, gait and overall functional independence.
• Apply specialised techniques such as Neuro‑Developmental Treatment (NDT) and Proprioceptive Neuromuscular Facilitation (PNF) where appropriate.
• Collaborate with physicians, nurses, occupational therapists and speech therapists to ensure holistic patient care.
• Monitor patient progress, evaluate outcomes and adjust treatment plans in line with clinical findings and patient goals.
• Educate patients and families on rehabilitation objectives, mobility strategies and discharge planning.
• Maintain accurate, timely clinical documentation in accordance with organisational and regulatory standards.
• Participate in multidisciplinary meetings and quality improvement initiatives.
