Registered Nurse – Post‑Acute Rehabilitation

About the role

The Registered Nurse – Post‑Acute Rehabilitation provides specialized nursing care to patients recovering from illness, injury, surgery, or neurological conditions, supporting their rehabilitation, functional independence, and safe discharge.

Key responsibilities

• Conduct comprehensive nursing assessments and develop individualized care plans for long‑term and transitional care patients.
• Provide advanced nursing care, close monitoring, and support for medically complex, ventilated, or severely disabled patients, ensuring best practices and patient safety.
• Administer medications and assist with complex medical procedures following clinical guidelines and unit protocols.
• Document care accurately and communicate effectively with the multidisciplinary team to ensure continuity and escalation of care.
• Participate in quality improvement, audits, and educational initiatives to enhance patient outcomes.

Required profile

• Bachelor’s degree in Nursing or equivalent.
• Valid medical license from the Department of Health (DOH) as a Registered Nurse or eligibility to obtain one.
• 2‑3 years experience in a Post‑Acute Rehabilitation Unit.
• Strong critical‑thinking and decision‑making skills under pressure.

Required skills

• BLS certification.
• ACLS certification.
• PALS certification (advantage).
