Case Coordinator / Nurse Case Manager

About the role

The Case Coordinator / Nurse Case Manager will work within a multidisciplinary team to ensure timely, high‑quality patient care. Reporting to the Senior Case Coordinator, the role bridges clinical, administrative and financial functions to optimise the patient’s episode of care.

Key responsibilities

• Maintain active communication with physicians, nursing staff and other multidisciplinary team members.
• Coordinate providers and services to progress patient care efficiently.
• Manage evidence‑based medical necessity screening and liaise with payers.
• Determine appropriate level of care placement and conduct utilization management.
• Provide accurate medical and health‑insurance documentation.
• Facilitate resource utilisation to match funding with clinical needs.
• Act as liaison between clinicians and the revenue department.
• Identify and resolve variances to clinical pathways and discharge obstacles.
• Support physicians with clinical documentation impacting case‑mix index and severity of illness.
• Monitor length of stay, resource use and report key indicators such as LOS, excess days and readmission rates.
• Advise patients and families, addressing psychosocial needs and delivering education.

Required profile

• Bachelor’s degree in Nursing (minimum two‑year program).
• Registered Nurse licensure from Canada, USA, UK, Ireland, South Africa, New Zealand or Australia.
• Current valid national nursing license.
• Basic Life Support (BLS) certification.
