Case Coordinator – Nurse Case Management

About the role

The Case Coordinator will work within a multidisciplinary team at Sheikh Shakhbout Medical City to ensure timely, high‑quality patient care. Reporting to the Senior Case Coordinator, you will coordinate providers, manage utilization, and act as a liaison between clinical and revenue departments.

Key responsibilities

• Maintain active communication with physicians, nurses and other team members to manage patient care.
• Coordinate appropriate providers and services to progress each patient’s episode of care.
• Apply evidence‑based medical necessity criteria for admissions, continued stay reviews and payer communications.
• Determine correct level of care placement and manage utilization of resources.
• Provide accurate medical and health‑insurance documentation.
• Identify and resolve variances to clinical pathways and discharge obstacles.
• Monitor length of stay, resource use and report key indicators such as LOS, excess days and readmission rates.
• Advise and educate patients and families based on identified learning needs.

Required profile

• Registered Nurse or equivalent clinical qualification.
• Experience in case management or utilization management within a hospital setting.
• Strong communication skills and ability to work collaboratively with multidisciplinary teams.
• Knowledge of health‑insurance processes and medical documentation requirements.
• Ability to assess psychosocial needs and coordinate with social workers.
