Nurse Patient Care Coordinator – Gastroenterology

About the role

Coordinates and handles care for patients receiving outpatient gastroenterology treatment, assessing needs, scheduling appointments, providing education and support, and ensuring continuity of care.

Key responsibilities

• Provides direct patient care, including medication administration, vital sign monitoring, and assistance with treatments.
• Conducts comprehensive client assessments to inform optimal discharge planning.
• Coordinates care plans and treatment schedules with healthcare professionals.
• Educates patients and families about diagnoses, treatment options, side effects, and offers emotional support.
• Maintains accurate medical records and documentation in the EMR.
• Advocates for patients' needs within the healthcare system and participates in patient‑related meetings.
• Participates in clinical research trials and quality improvement initiatives.

Required profile

• Bachelor’s degree in nursing (or equivalent) and registration as a Registered Nurse from Canada, USA, UK, Ireland, South Africa, New Zealand, or Australia.
• Minimum five years of recent post‑graduate experience in the related specialty.
• Valid license/registration to practice in home country and Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification.
• Knowledge of age‑appropriate care, assessment of physical and psychological needs, and care coordination.
