Outpatient Medical Coder

About the role

The Outpatient Medical Coder is responsible for reviewing and translating outpatient clinical documentation into accurate ICD‑10‑CM, CPT and HCPCS codes. This role supports billing, reimbursement and compliance while ensuring high coding quality and productivity.

Key responsibilities

• Review physician notes, diagnostic reports and other outpatient records to assign correct diagnosis and procedure codes.
• Apply appropriate modifiers, ensure proper sequencing of codes and verify that documentation meets coding guidelines.
• Identify missing or unclear documentation and coordinate queries with clinical staff.
• Participate in internal audits, quality reviews and coding education activities.
• Support denial management by providing accurate coding information and corrective actions.

Required profile

• Diploma or Bachelor’s degree in Medical Coding, Health Information Management or related field; coding certification (CPC, CCS, CCA) preferred.
• Knowledge of outpatient coding guidelines, ICD‑10‑CM, CPT and HCPCS systems, and medical terminology.
• Previous experience in outpatient medical coding is preferred.

Required skills

• Proficiency with ICD‑10‑CM, CPT and HCPCS coding systems.
• Experience using EMR/EHR platforms and coding software.
• Strong understanding of medical terminology and clinical documentation.
• Ability to maintain confidentiality and adhere to privacy standards.
