Registered Nurse – Post‑Acute Rehabilitation

About the role

The Registered Nurse – Post Acute Rehabilitation provides specialized nursing care to patients recovering from illness, injury, surgery, or neurological conditions. The role supports rehabilitation, functional independence, and safe discharge through close collaboration with a multidisciplinary team.

Key responsibilities

• Conduct comprehensive nursing assessments and develop individualized care plans for long‑term and transitional care patients.
• Provide advanced nursing care, close monitoring, and support for medically complex, ventilated, or severely disabled patients.
• Administer medications and assist with complex medical procedures in line with clinical guidelines and unit protocols.
• Document care accurately, communicate effectively with the multidisciplinary team, and participate in quality improvement initiatives.

Required profile

• Bachelor’s degree in Nursing or equivalent.
• Valid DOH medical license as a Registered Nurse or eligibility to obtain one.
• 2–3 years experience in a Post‑Acute Rehabilitation Unit.
• BLS and ACLS certifications required; PALS is an advantage.
• Strong critical‑thinking and decision‑making skills under pressure.
