CMO - Discharge Coordinator

About the role

The CMO - Discharge Coordinator supports safe and efficient patient transitions within the Amana Healthcare network, part of M42. Reporting to the Case Manager, the role leverages a licensed nursing or medical background to coordinate discharge planning, ensure continuity of care, and communicate with multidisciplinary teams, patients, families, insurers, and external facilities.

Key responsibilities

• Coordinate and manage patient discharges to ensure safe, timely, and efficient transitions of care.
• Support case management activities by assessing patient needs and planning appropriate discharge pathways.
• Monitor patient flow and collaborate with clinical teams to maintain continuity of care across the hospital.
• Communicate effectively with multidisciplinary teams to align discharge plans and resolve barriers.
• Liaise professionally with patients, families, insurance providers, and external healthcare facilities.
• Maintain accurate documentation in EMR systems and ensure compliance with regulatory and quality standards.
• Prioritize and manage multiple discharge cases simultaneously in a fast‑paced environment.
• Provide culturally sensitive support, with Arabic language communication preferred.

Required profile

• Licensed Nurse with a valid professional nursing license or a relevant medical background.
• 2‑3 years of experience in discharge planning, case management, care coordination, or a similar healthcare role.
• Proficiency in EMR systems and Microsoft Office.
• Strong understanding of healthcare regulations and documentation standards.
• Excellent communication, negotiation, and interpersonal skills; Arabic language proficiency preferred.
