Case Coordinator / Nurse-Case Management

About the role

The Case Coordinator / Nurse-Case Management role supports the multidisciplinary team in delivering high‑quality patient care at SSMC. Working under the Senior Case Coordinator, you will coordinate providers, manage utilization, and ensure timely progression of each patient’s episode of care.

Key responsibilities

• Maintain active communication with physicians, nurses and other team members to manage patient care.
• Manage evidence‑based medical necessity screening, admission reviews and payer communications.
• Determine appropriate level of care placement and oversee utilization management.
• Provide accurate medical and health‑insurance documentation and act as liaison between clinicians and the revenue department.
• Identify and resolve variances to clinical pathways, facilitate discharge planning and coordinate continuum of care.
• Monitor length of stay, resource use and report indicators such as LOS, excess days and readmission rates.
• Support physicians with clinical documentation impacting case mix index and severity of illness.
• Advise patients and families, addressing psychosocial needs and delivering education as required.

Required profile

• Bachelor’s Degree in Nursing and a current national nursing license (Canada, USA, UK, Ireland, South Africa, New Zealand or Australia).
• Basic Life Support (BLS) certification.
• Case Management Certification.
• Minimum 6 years nursing experience at SSMC or at least 2 years in case management within a similar healthcare facility.
• Experience in a large healthcare facility is desirable.
