Clinical Social Worker

About the role

The Clinical Social Worker will join Bareen International Hospital’s multidisciplinary team to assess and support patients’ psychosocial needs throughout their hospital stay. This role involves collaborating with medical staff to develop comprehensive care plans and ensuring smooth transitions to post‑hospital services.

Key responsibilities

• Conduct thorough assessments of patients’ mental, emotional, and social circumstances, including family dynamics, financial status, and support networks.
• Develop and implement individualized care or discharge plans in coordination with doctors, nurses, and allied health professionals.
• Provide counseling and emotional support to patients and families facing stress, trauma, chronic illness, or end‑of‑life situations.
• Coordinate discharge processes, arranging rehabilitation, home health services, or placement in long‑term care facilities.
• Connect patients with community resources, financial aid programs, housing services, and support groups.
• Advocate for patients’ rights and assist them in navigating healthcare systems and insurance procedures.
• Respond to crisis situations such as abuse cases, mental‑health emergencies, or sudden illness.

Required profile

• Bachelor’s or Master’s degree in Social Work (BSW/MSW).
• Professional social work licensing appropriate to the region.
• Experience in healthcare or clinical settings is preferred.
