OP Medical Coder – Immediate Joiner

About the role

The OP Medical Coder is responsible for accurately reviewing and coding outpatient medical records, diagnoses, procedures, and services. This role ensures that documentation is translated into correct ICD‑10‑CM, CPT, HCPCS, and applicable modifiers to support billing, reimbursement, and compliance. Immediate joiners are expected to quickly integrate with the coding team and meet productivity standards.

Key responsibilities

• Review outpatient clinical documentation, physician notes, and diagnostic reports.
• Assign accurate diagnosis and procedure codes using ICD‑10‑CM, CPT, HCPCS, and appropriate modifiers.
• Identify missing or unclear documentation and coordinate queries with clinical staff.
• Ensure coding compliance with payer guidelines, correct sequencing, and support denial management.
• Participate in internal audits, quality reviews, and stay updated on coding guideline changes.

Required profile

• Diploma or Bachelor’s degree in Medical Coding, Health Information Management, or related field.
• Professional coding certification (CPC, CCS, CCA) preferred.
• Knowledge of outpatient coding guidelines, medical terminology, anatomy, and common procedures.
• Previous outpatient coding experience is advantageous.
