Clinical Pharmacist - TP

About the role

The Clinical Pharmacist provides patient care within the hospital, collaborating with physicians and healthcare professionals on medication therapy. The role involves evaluating medication regimens, advising on safe and cost‑effective drug use, and participating in education and quality improvement initiatives.

Key responsibilities

• Conduct medication therapy evaluations, provide recommendations, and monitor adverse drug reactions.
• Participate in patient care rounds, develop interdisciplinary care plans, and advise physicians on drug selection and cost‑effectiveness.
• Act as a drug information resource, educate patients and staff, and contribute to antibiotic stewardship and medication safety programs.
• Prepare and submit ADR reports, review laboratory data, and ensure documentation of therapeutic recommendations.

Required profile

• Doctor of Pharmacy (Pharm‑D) or Master’s in Pharmacy from an accredited university.
• Current clinical pharmacy license and successful completion of a recognized residency or post‑graduate clinical pharmacy qualification.
• Minimum three years of post‑certification experience as a Clinical Pharmacist in a hospital setting.
