Nurse Patient Care Coordinator – Gastroenterology

About the role

The Nurse Patient Care Coordinator will manage outpatient gastroenterology patients, ensuring seamless care from assessment through treatment and follow‑up. This role combines direct nursing care with patient education, coordination of appointments, and collaboration with multidisciplinary teams.

Key responsibilities

• Provide direct patient care, administer medications, monitor vital signs, and assist with treatments.
• Conduct comprehensive assessments covering functional, environmental, psycho‑social, and health information to support optimal discharge planning.
• Coordinate care plans and treatment schedules with physicians, nurses, and other healthcare professionals.
• Educate patients and families about diagnoses, treatment options, side effects, and provide emotional support.
• Maintain accurate medical records and document all patient interactions in the Electronic Medical Records (EMR) system.
• Advocate for patient needs, participate in patient meetings, and support patients throughout clinical research trials.
• Contribute to quality improvement initiatives and departmental goal setting.

Required profile

• Bachelor’s degree in nursing (minimum three‑year program) or equivalent nursing degree.
• Registered Nurse qualification from Canada, USA, UK, Ireland, South Africa, New Zealand, or Australia.
• Relevant clinical experience in outpatient or gastroenterology settings.
• Strong communication skills and ability to work collaboratively with patients, families, and healthcare teams.
