Patient Care Coordinator - OPD Medical

About the role

The Patient Care Coordinator is a registered nurse who acts as the patient's named clinical key worker from referral through treatment, follow-up, survivorship or transition to palliative and supportive care. The role covers specialist nursing assessment, care coordination, patient and family education, symptom and risk screening, clinical triage, proactive follow-up and timely escalation with the consultant, multidisciplinary team and support services. It is a full-time position based in Abu Dhabi.

Key responsibilities

• Coordinate the patient's end-to-end clinical pathway and maintain regular contact with patients and families
• Facilitate access to support services such as finance and insurance, psycho-social counselling, social work, nutrition, rehabilitation, palliative care and allied health
• Screen for health literacy using a standardized tool and refer patients to community resources such as transport assistance, accommodation and patient support groups
• Identify barriers to care (logistical, financial, cultural, linguistic, health literacy) and escalate them for resolution
• Provide culturally sensitive support, arranging interpreters where needed
• Collect and analyse pathway data (referral volumes, appointment access, time to treatment) and evaluate progress against outcomes
• Develop and implement care plans with the patient, family and multidisciplinary team, and adjust them as needed
• Evaluate practice against standards and regulations, and share knowledge with colleagues
• Support outreach and awareness activities that bring patients earlier into care

Required profile

• Registered nurse with specialist nursing assessment, care planning, clinical triage and symptom screening within competence
• Ability to communicate positively with patients, families and colleagues, and to act as a role model and mentor
• Knowledge of current practice standards, including NHS Personalised Care, CMSA and patient navigation standards
