Patient Care Coordinator – Operations Management

About the role

The Patient Care Coordinator acts as the named clinical key worker for patients throughout their entire care pathway—from referral to treatment, follow‑up, survivorship or transition to palliative care. Working closely with consultants and multidisciplinary teams, the coordinator provides specialist nursing assessment, education, symptom screening, clinical triage and proactive follow‑up to ensure safe, timely and coordinated care.

Key responsibilities

• Coordinate the end‑to‑end clinical pathway, arranging investigations, appointments and follow‑up while maintaining regular contact with patients and families.
• Facilitate shared decision‑making and promptly communicate any changes in clinical status or pathway delays to the accountable clinical team.
• Serve as the primary liaison to support services such as finance, insurance, psychosocial counselling, social work, nutrition, rehabilitation and palliative care.
• Perform specialist nursing assessments, care planning, clinical triage and symptom/toxicity screening within scope of practice.
• Collect, analyse and report pathway performance data (e.g., referral volumes, time‑to‑treatment) to drive quality improvement.
• Develop individualized care plans in partnership with patients, families and the multidisciplinary team, and adjust plans based on ongoing evaluation.

Required profile

• Registered Nurse with experience in specialist assessment and care coordination.
• Ability to work across organisational and professional boundaries to promote continuity and safety of care.
• Strong communication and collaboration skills with multidisciplinary teams, patients and families.
• Knowledge of professional standards, regulatory requirements and escalation procedures.
• Analytical mindset for data collection, performance monitoring and process improvement.
