Patient Care Navigator

About the role

As a Patient Care Navigator you will be responsible for organizing and coordinating patient care activities to ensure optimal outcomes. You will collaborate with healthcare providers, patients and families to facilitate communication, address concerns and resolve issues, acting as a liaison to create a seamless and positive patient experience.

• Resolve issues that may affect patient experience.
• Act as liaison between patients, families and the healthcare team.
• Supervise the patient care team and uphold clinical standards.
• Promote health education and deliver age‑appropriate patient care.
• Support and promote SEHA’s mission, vision and values.

Required profile

• Demonstrate professional leadership in supervising patient care teams.
• Adhere to the Professional Nurses Code of Conduct.
• Provide age‑appropriate care aligned with the Nursing Scope of Practice.
• Commit to upholding clinical quality standards within the specialty.
• Promote health education and community wellbeing.
