Registered Nurse – Remote Patient Monitoring

About the role

We are seeking a Registered Nurse to join our remote patient monitoring team. The nurse will use digital health tools to oversee patients’ health status from a central location, ensuring timely clinical interventions. This role supports chronic disease management, promotes medication adherence, and provides health education to patients remotely.

Key responsibilities

• Monitor patients remotely via RPM platforms, reviewing vitals, trends, alerts, and clinical data.
• Identify potential health concerns and conduct remote clinical assessments.
• Provide nursing interventions promptly based on assessment findings.
• Escalate critical findings and coordinate care with physicians and multidisciplinary teams.
• Support patients in chronic disease management, medication compliance, and health education.
• Document patient interactions and maintain accurate electronic health records.
• Participate in virtual multidisciplinary case conferences.
• Educate patients on the proper use of monitoring devices and digital health applications.

Required profile

• Bachelor’s degree in Nursing.
• Minimum 3 years of nursing experience.
• Fluent in English.
• Experience with remote patient monitoring or telehealth preferred.
• Ability to work independently and make clinical decisions.

Required skills

• Proficiency with digital health and remote patient monitoring platforms.
• Knowledge of clinical protocols for chronic disease management.
