About the role
We are seeking an OP Medical Coder to review physician documentation, medical records, and outpatient encounters, ensuring accurate coding and compliance.
Key responsibilities
- Assign accurate ICD-10-CM, CPT, and HCPCS codes to outpatient services.
- Validate diagnoses, procedures, modifiers, and medical necessity.
- Identify missing, unclear, or conflicting documentation and escalate queries.
- Review coding edits, denials, and documentation-related issues.
- Maintain productivity and accuracy targets while following HIPAA and organizational compliance.
Required profile
- Knowledge of ICD-10-CM, CPT, and HCPCS coding systems.
- Understanding of outpatient/ambulatory coding guidelines.
- Medical terminology and anatomy knowledge.
- Strong attention to detail and analytical skills.
- Certification such as CPC, CCS, CCA or equivalent is often preferred.
