Hospital Pharmacist (Inpatient)

About the role

A hospital is seeking a pharmacist to provide clinical pharmacy services in its inpatient setting. The role covers medication review, dispensing, patient education, medication safety and collaboration with psychiatrists, nurses and other healthcare professionals.

Key responsibilities

• Clinical pharmacy: review and verify medication orders, conduct medication reconciliation at admission, transfer and discharge, and monitor patients for effectiveness, adverse reactions and drug interactions.
• Take part in multidisciplinary clinical rounds and give pharmacotherapeutic recommendations to psychiatrists, physicians and other healthcare professionals, optimising psychotropic medication therapy according to clinical guidelines.
• Medication management: dispense accurately, ensure proper storage, labelling and distribution, maintain inventory, manage controlled medications and coordinate procurement of formulary drugs.
• Educate patients and families about their medicines, promote adherence and provide discharge counselling.
• Medication safety: take part in safety initiatives, audits and quality improvement, report and investigate errors and adverse events, help develop policies and support accreditation and compliance.
• Collaborate with psychiatrists, nurses, psychologists and physicians, give drug information, take part in case reviews and committees, and support staff training.

Required profile

• Bachelor of Pharmacy (B.Pharm) or Doctor of Pharmacy (PharmD) from an accredited institution.
• Valid Department of Health (DoH) license or eligibility as a pharmacist.
• At least 3-5 years of hospital inpatient pharmacy experience; inpatient pharmacy experience is mandatory.
• Mental health or behavioural health hospital experience is an advantage.
